If you missed the original communication with regard to this reporting, the following is the original email we sent out on January 5th. The Office of School Support and Family Engagement also mailed a hardcopy of the notice to district superintendent s office on the same date. The forms are very easy to complete, but if additional assistance is needed, you may send your staff to our Feb 1st workshops. We still have some seats available. Please feel free to contact us for an additional questions regarding this annual report (see contact information in original email). Thank you. ======= original notice sent out on January 5th ======== The Annual School Health Report has been converted to a web-based data entry process, part of the E-RIDE system (http://www.eride.ri.gov). This will simplify the process of completing the form. Each district’s form will be pre-populated with last year’s information allowing for updates as necessary. There are some minor changes to the form from the previous year, to ensure alignment with new statutes, regulations or standards of care. It is important to have appropriate staff review and complete specific sections of the form, e.g. school nurse teacher, facilities manager, health educator, chemical hygiene officer, etc. YOUR DISTRICT DATA MANAGER/ERIDE ADMINISTRATOR WILL NEED TO CREATE/DESIGNATE APPROPRIATE USER ACCOUNTS FOR YOUR STAFF TO COMPLETE THE FOMR ON ERIDE.
